同種造血細胞移植は組織適合性抗原の異なった他者の造血幹細胞をいわば人工的に移植する治療法である．ドナー由来の造血幹細胞から分化した免疫担当細胞が患者の正常組織に障害を与える移植片対宿主病と患者の白血病細胞を傷害する移植片対白血病効果などの同種移植免疫反応にはHLAが重要な役割を果たす．非血縁者間骨髄移植が実施された多数例のドナーと患者のHLA情報と臨床成績を後方視的に解析し，同種造血幹細胞移植におけるHLAの意義が明らかになっている知見と今後の展望について概説する．
